Searching for a name, whether as a Party or a Provider, can be accomplished by selecting any of the name criteria in the dropdown.
The dropdown contains fields from the Party and Provider tabs that can be used as search criteria. If you know information about a name, you can retrieve their Neos entry using the following criteria:
Select the desired criteria option and enter text in the Search field. Click the Search icon
or hit the ENTER key to perform the search.
Name/AKA
Enter a fragment of a Name or AKA name in the Search field. A name may be involved in several Cases or Intakes. A Party can also be entered as Provider name in another Case. In both scenarios, you need to ensure you select the appropriate Case or Intake from the search results. A name can be located when only the middle initial (no period) is entered in the Middle field on the Name Details window. Refer to Using Name/Aka Search Criteria for more details.
SSN
A name can be retrieved using the full or partial first three, middle two, or last four digits of a Social Security Number. The Search text field already contains the appropriate formatting, so dashes do not need to be entered.
A name can be retrieved using a fragment of the linked Email address.
Name ID
The Name ID is a unique number that is automatically assigned to a name by Neos when it is entered into the system. The number is displayed in the detail view of the Party or Provider tabs. Any matching names can be retrieved using a fragment of the Names ID.
Any Phone
A name can be retrieved using the complete or partial Phone number. The Search field already contains the appropriate formatting for a phone number, so you do not need to enter the parentheses or dashes.
City
A name can be retrieved using the complete or partial City name.
Zip Code
A name can be retrieved using the complete or partial Zip Code.
DOB (Date of Birth)
A name can be retrieved using the complete or partial Date of Birth. Click the calendar icon
to select a date.
The Search field contains the default date format, so you do not need to enter a slash in your search.
Federal ID
A name can be retrieved using the complete or partial Federal ID number. The Search field contains the default Federal ID format, so you do not need to enter a dash in your search.
