This article describes the process for performing a Medical Record Extraction. Medical Record Extractions utilize standard and custom dynamic layouts in the Related Items tab. Data extraction for Medical Records has been improved, allowing data from the full document to be structured into a cleaner, standardized format for the associated dynamic layouts.

NOTE: Your firm must have NeosAI a user must have the NeosAI Special Permission enabled.
NeosAI Extractions can be performed on any document based on the supported file type. When the Document Type is set to "Medical Record" with a supported file type of PDF, TXT, or DOCX, the Analyze with NeosAI window will default to Medical Data and Contact Information. The summary generation check box will also be automatically selected.
Select the Analyze button. The Processing window will display on the bottom right of Neos. Click the down arrow to expand the actions that are in progress. If Generate a summary was selected, the progress bar for AI Summary will display. [screenshot]
When complete, the document will have a checkmark in the AI Extract column.
On the detail view of the document, an "AI Enhanced" tag will display with a new Related Items tab to show the extracted data from your file as it relates to the Dynamic Layouts Directory. The Medical Record Document Type contains a standard Dynamic Layout and columns.

When the analysis is complete, dynamic layouts will be created for each Medical Condition, Treatment, Examination, and Medication.
Additional standard Dynamic Layouts and standard columns can be inserted into the Related Items tab by clicking the + New Entry button on the top right of the desired grid section (e.g., Examinations, Medical Conditions, etc.). For example, to add additional medications, click the + New Entry button.
Enter data into at least one field and click the Save button.
A line item for the entered "Medication" will be included in the Related Items tab. The Date Created and Date Modified columns will show the date and time the record was added to Neos using the Time Zone set in the Staff Profile.
To edit an entry, single-click the entry to access the preview pane where existing fields can be modified.
Double-clicking the entry will open the detail view where Extraction Details (including Medical Codes) can be accessed, data modifications can be made, and additional Dynamic Layouts can be added as a subset of your selected entry.
Extraction Details
Dynamic Layout fields can have extra metadata which can now be accessed when opening the dynamic layout. The extra info button is accessible on the preview pane and detail view of a single entry.

When selected, the button will change to solid blue and display an Extraction Details popup.
The Extracted Value field displays the original extracted data from the medical record document. The From Document field includes the document and file type.
The Extracted Value will be shown in the original context of the document, up to 100 characters before and after that value. Click the Expand button to increase the original context and show a total of 200 characters. Click the Collapse button to return the document context to its original size. NOTE: The original context of the document will not retroactively display on documents uploaded without a stored OCR (prior to the September 2024 release). [removed feature – will be added later]
By clicking the linked document, the Extraction Details popup will close and open the preview pane of that document.
The Medical Codes pertinent to the particular medical field are displayed. Click the copy button to copy a code to your clipboard. With access to UMLS, ICD, CPT, and DXP codes, users can save time from having to look up medical codes separately.
